Pathways to Health Longevity through
Lifelong Health and Well-Being Strategies

Health longevity, which is defined as the ability to live longer lives in good health, free from disability and chronic illness, is increasingly recognized as a global public health priority. With the world’s population aging rapidly, ensuring healthy, productive, and independent lives across the lifespan is central to the World Health Organization (WHO) Decade of Healthy Ageing (2021-2030) and achieving Sustainable Development Goal (SDG) 3, which aims to “ensure healthy lives and promote well-being for all at all ages.” Health longevity is not just about extending life expectancy but also about improving an individual’s healthspan – the period of life spent in optimal health. Achieving health longevity requires a multisectoral approach, including preventive healthcare, chronic disease management, nutrition, physical activity, social engagement, and equitable access to healthcare services. Advances in precision medicine, digital health, and anti-aging research provide new opportunities for extending health longevity, while challenges such as health inequities, chronic disease burdens, and ageism must be addressed. This Fast-Track Health policy brief highlights best practices through five case studies, explores challenges and opportunities, and provides actionable recommendations for municipal, national, and regional health authorities.

CASE STUDIES

Community-Based Longevity in Okinawa, Japan: Japan’s Okinawa Prefecture is one of the world’s five Blue Zones—regions with exceptionally high life expectancy and low rates of chronic disease. Longevity in Okinawa is attributed to plant-based diets, physical activity, strong social networks, and purpose-driven living (ikigai). Government policies supporting elderly community engagement, traditional diets, and preventive healthcare have reinforced these trends. Okinawa’s model highlights the importance of social and environmental factors in promoting health longevity. (National Geographic, 2023)

Precision Medicine for Longevity in the United States: The All of Us Research Program in the United States, led by the US National Institutes of Health (NIH), is integrating genomics, digital health data, and personalized medicine to tailor healthcare for disease prevention and longevity. By analyzing genetic, lifestyle, and environmental factors, the program is developing personalized treatment plans for aging-related diseases, such as Alzheimer’s, cardiovascular disease, and cancer. The initiative demonstrates how precision medicine can optimize longevity and health span. (NIH, 2023)

Nordic Lifelong Physical Activity Promotion: In Denmark, Sweden, and Finland, governments have adopted lifelong physical activity policies to promote healthy aging and longevity. Investments in public sports facilities, city-wide walking and cycling infrastructure, and subsidized fitness programs for older adults have contributed to lower rates of frailty, obesity, and cardiovascular diseases. Research shows that maintaining physical activity in older age extends health span and reduces disability. The Nordic model highlights the role of active lifestyles in promoting longevity. Healthy Longevity Policies in Singapore: Singapore’s Action Plan for Successful Ageing integrates preventive health, age-friendly urban planning, financial security, and lifelong learning to extend health longevity. The country has expanded screening programs for chronic diseases, promoted digital literacy among older adults, and restructured cities for active aging. As a result, life expectancy in Singapore has surpassed 84 years, with rising healthy life expectancy. Singapore’s experience highlights how integrated policies can extend both lifespan and health span. (WHO Western Pacific, 2023)

Longevity Research and Policy in Switzerland: The Swiss government has invested in longevity-focused biomedical research, including stem cell therapies, regenerative medicine, and AI-driven predictive health analytics. Switzerland’s mandatory universal healthcare system ensures that older adults receive comprehensive preventive care, regular screenings, and access to cutting-edge treatments. Additionally, Switzerland has implemented age-friendly city policies that support social inclusion, intergenerational housing, and walkable urban environments to enhance older adults’ quality of life. CHALLENGES AND OPPORTUNITIES

Advancing health longevity faces several systemic and societal challenges. Chronic diseases, including cardiovascular disease, diabetes, and neurodegenerative disorders, remain the leading barriers to longevity, with many health systems still focused on reactive care rather than prevention. Health inequities exacerbate disparities in longevity, with low-income and marginalized populations experiencing shorter healthspans due to limited access to healthcare, healthy foods, and physical activity opportunities. Ageism in healthcare and employment further marginalizes older adults, restricting their access to innovative treatments and financial security. The rising cost of healthcare and long-term care services poses economic challenges for both individuals and governments, particularly as aging populations grow. Additionally, climate change and environmental degradation increasingly impact longevity by exacerbating air pollution, heat-related illnesses, and food insecurity, posing new threats to global aging populations. Gaps in research on anti-aging therapies and regenerative medicine also limit the rapid translation of scientific breakthroughs into public health policies.

Despite these challenges, significant opportunities exist to extend health longevity through preventive health strategies, innovation, and public policy. Expanding universal health coverage (UHC) to include preventive screenings, geriatric care, and chronic disease management can reduce health inequities and improve life expectancy. The integration of AI, telemedicine, and wearable health technology provides new tools for early disease detection, lifestyle tracking, and personalized health interventions. Policies that encourage lifelong physical activity, healthy diets, and smoking cessation can significantly improve population-wide longevity outcomes. Governments can promote age-friendly urban planning, including walkable cities, accessible healthcare facilities, and intergenerational housing, to support active aging. Research into regenerative medicine, gene therapy, and cellular rejuvenation holds promise for extending healthspan and delaying age-related diseases. Finally, multisectoral collaboration between public health, urban planning, and technology sectors can develop holistic solutions for promoting longevity and improving quality of life in aging populations.

FAST-TRACK HEALTH RECOMMENDATIONS

For municipal health authorities:

  • Implement community-based longevity programs (e.g., fitness initiatives, senior wellness hubs).
  • Expand urban planning to promote walkability, public transportation access, and green spaces.
  • Promote healthy nutrition education/accessibility through public health campaigns/subsidies.
  • Use intergenerational programs to reduce ageism/foster cross-generational knowledge-sharing.
  • Invest in local telehealth services to improve chronic disease management and access to specialist care for older adults.

For national health authorities:

  • Strengthen preventive health policies (e.g., screenings, vaccinations, lifestyle interventions).
  • Expand UHC to include age-related conditions, rehabilitation services, and home-based care.
  • Support research into longevity science, including precision medicine and gene therapies.
  • Develop long-term care financing mechanisms, ensuring sustainability in aging populations.
  • Promote healthy aging through national physical activity and dietary guidelines and policy.

For regional health authorities:

  • Establish cross-border research collaborations on ageing-related diseases and health enablers.
  • Use regional nutrition/physical activity policies to encourage longevity-promoting behaviors.
  • Create shared longevity regional data networks to monitor aging trends and health outcomes.
  • Support regional procurement strategies for age-related medical and other technologies.
  • Advocate policy frameworks integrating longevity science into economic/public health planning.
