Mitigating Climate-Driven Impacts on
Pulmonary and Respiratory Health
Pulmonary and respiratory health are closely related but distinct domains. Respiratory health refers broadly to the health of the respiratory system—including the airways, lungs, and related structures involved in breathing—covering conditions such as asthma, COPD, lung infections, and lung cancer. Pulmonary health is more specific, focusing on the lungs and lung tissue themselves, where conditions like pulmonary fibrosis, COPD, and pulmonary hypertension manifest most directly. Climate change intensifies risks to both domains, as air pollution, wildfire smoke, rising allergen levels, and extreme temperature events contribute to respiratory system irritation, lung inflammation, and the progression of chronic pulmonary diseases. Globally, the World Health Organization (WHO) reports that more than 3.23 million deaths annually are attributed to COPD and 1.6 million to lung cancer, underscoring the scale of the challenge. Climate-driven exposures increase vulnerability, especially for children, older adults, and individuals with pre-existing conditions. Aligning with SDG 3 (reducing premature mortality from NCDs and communicable diseases) and SDG 13 (climate action), integrating climate adaptation and mitigation strategies into pulmonary and respiratory health programming is an urgent global health priority. This Fast-Track Health policy note highlights best practices through five case studies, highlights challenges and opportunities, and provides actionable recommendations for municipal, national, and regional health authorities.
CASE STUDIES
Flood Resilience in Bangladesh: Finland’s National Asthma Program reduced asthma-related hospitalizations by 54% within a decade through early diagnosis, patient education, and guideline-based care. The program also emphasized self-management with action plans and improved healthcare provider training. As rising pollen counts and allergen exposure intensify with climate change, Finland has incorporated adaptation strategies such as predictive pollen mapping and urban greening to mitigate asthma triggers. Pneumonia Vaccination Campaign in Rwanda: Rwanda’s pneumococcal vaccination program for children under five dramatically reduced pneumonia incidence and mortality. Through partnerships with Gavi, the government secured vaccine affordability and equitable access nationwide. In light of climate-related weather fluctuations that heighten pneumonia risks, Rwanda has targeted vaccination campaigns toward high-risk areas and integrated community awareness on climate-linked disease patterns. Lung Cancer Screening in the United States: Low-dose computed tomography (LDCT) screening for high-risk groups in the U.S. reduced lung cancer mortality by 20%, emphasizing early detection. Coverage integration through insurance has expanded access, especially for long-term smokers. Amid growing concerns about wildfire-related particulate matter, ongoing studies are examining how climate-driven exposures interact with pulmonary carcinogenesis, linking cancer prevention with environmental health. Air Quality Improvement in Germany: Germany’s National Air Pollution Control Program reduced PM2.5 by 35% and NO2 emissions by 40% between 2000 and 2020 by phasing out coal, adopting renewable energy, and enforcing strict vehicle emissions standards. Investments in sustainable urban mobility and clean energy were central to success. These efforts demonstrate that addressing the root causes of air pollution both protects respiratory health and advances climate mitigation goals simultaneously. (European Environment Agency, 2023)
CHALLENGES AND OPPORTUNITIES
Efforts to address pulmonary and respiratory health in the era of climate change face persistent barriers. Limited access to diagnostics and essential medicines—particularly in low- and middle-income countries—undermines effective management of chronic and acute conditions. Climate change intensifies environmental exposures, from wildfire smoke to pollen surges, further straining vulnerable populations. Structural health inequities mean that rural and low-income groups often lack access to care, heightening disparities in outcomes. Enforcement of environmental regulations remains inconsistent, slowing transitions to clean energy. At the same time, stigma and lack of awareness delay early intervention for lung diseases, while insufficient surveillance data on combined climate and respiratory risks hinder policymaking and funding allocation.
Despite these challenges, significant opportunities exist to integrate climate adaptation and mitigation into pulmonary and respiratory health strategies. Expanding smoking cessation and anti-tobacco policies will reduce disease burdens while aligning with environmental protection goals. Vaccination against respiratory infections such as pneumonia and influenza can build resilience in populations most affected by climate-related risks. Air quality improvement initiatives, including renewable energy adoption and urban greening, directly lower exposures while advancing climate action. Digital health technologies enable remote respiratory monitoring and early warning systems during pollution or wildfire events. Global advocacy linking health and climate agendas creates momentum to secure financing for climate-resilient lung health programs, while emphasizing equity to ensure vulnerable populations benefit most.
