Prevention, Treatment, and Management of
Communicable Diseases to Achieve WHO Targets

Communicable diseases remain a significant global health challenge, accounting for millions of deaths annually, particularly in low- and middle-income countries. The World Health Organization (WHO) has prioritized reducing the burden of communicable diseases, including HIV, tuberculosis (TB), malaria, and emerging infectious diseases, through universal health coverage (UHC), strengthening health systems, and fostering global collaboration. This Fast-Track Health policy brief highlights opportunities for advancing the prevention, treatment, and management of communicable diseases, illustrated through five case studies, and provides actionable recommendations for municipal, national, and regional health authorities.

CASE STUDIES

HIV Prevention and Treatment in South Africa: South Africa, home to the largest HIV epidemic globally, has scaled up its prevention and treatment efforts to achieve the UNAIDS 95-95-95 targets by 2030. A combination of community outreach, pre-exposure prophylaxis (PrEP), and antiretroviral therapy (ART) expansion has significantly improved outcomes. Digital tools, including mobile apps, provide medication reminders and facilitate teleconsultations. Since 2015, South Africa has achieved a 40% reduction in new HIV infections and a 50% decrease in AIDS-related deaths. Key lessons include the importance of integrating HIV services into primary healthcare and addressing stigma through community-led interventions. (UNAIDS, 2023)

TB Control in India: India, which accounts for a quarter of global TB cases, implemented the National TB Elimination Program (NTEP) to reduce TB incidence. Innovations include AI-powered diagnostics, free treatment for all TB patients, and nutritional support to enhance treatment adherence. The program also introduced community-based health workers to monitor and support patients, resulting in a 15% increase in treatment success rates. Malaria Elimination in Sri Lanka: Sri Lanka became malaria-free in 2016, a milestone achieved through robust surveillance, vector control, and community engagement. The government provided free diagnostic and treatment services while health workers conducted door-to-door awareness campaigns in high-risk regions. Post-elimination, the country continues active surveillance to prevent reintroduction, underscoring the need for sustained commitment to surveillance and community involvement even after elimination goals are achieved. COVID-19 Response in Singapore: Singapore’s response to COVID-19 demonstrated how early preparedness and technology integration can mitigate disease impact. The country deployed digital contact tracing tools, mass testing, and a robust vaccination campaign. Specialized task forces ensured resource allocation, while public health campaigns emphasized preventive behaviors. These efforts resulted in low mortality rates and high vaccination coverage. The case illustrates the value of real-time data systems and community trust in pandemic responses. (Chong et al., 2021)

Hepatitis C Elimination in Egypt: Egypt, which previously had the highest global prevalence of hepatitis C, launched a nationwide screening and treatment program. The initiative provided free testing for over 60 million individuals and subsidized antiviral medications for those infected. Partnerships with pharmaceutical companies enabled reduced drug prices. Within five years, Egypt reported a 92% reduction in hepatitis C-related mortality. CHALLENGES AND OPPORTUNITIES

Combatting communicable diseases presents numerous challenges, particularly in low- and middle-income countries where health system capacity is often constrained. Limited infrastructure, including diagnostic facilities, laboratories, and healthcare workforce shortages, hampers the effective delivery of preventive and treatment services. Persistent inequities exacerbate disease burden on marginalized populations already facing significant barriers to accessing care. Emerging infectious diseases pose additional threats, stretching strained resources and demanding rapid, coordinated responses. Stigma, particularly around diseases like HIV and TB, undermines efforts to engage affected communities in prevention and care. And, fragmented global coordination and inconsistent implementation of donor-funded programs can lead to inefficiencies and duplication of efforts, while the lack of integrated health data systems limits the ability to track disease trends and measure intervention outcomes.

Despite these challenges, there are significant opportunities to accelerate progress in the prevention, treatment, and management of communicable diseases. Digital health technologies, such as AI-driven diagnostics, mobile health tools, and telemedicine, offer scalable solutions to enhance disease surveillance, improve treatment adherence, and increase access to care. Integrating communicable disease programs into universal health coverage frameworks strengthens health systems and ensures continuity of care for affected populations. Community leadership, particularly through the engagement of community health workers and local organizations, fosters trust and enables culturally tailored interventions that improve health outcomes. Multisectoral collaboration, including partnerships with environmental and education sectors, can address the broader social determinants of communicable diseases. Leveraging innovative financing mechanisms and global collaboration may also facilitate resource mobilization and knowledge sharing for a united front against communicable diseases.

For municipal health authorities:

  • Implement community-led outreach programs focusing on education and stigma reduction
  • Establish localized disease surveillance systems to track and respond to outbreaks in real time
  • Collaborate with schools and workplaces to promote vaccination and preventive behaviors
  • Use mHealth tools to enhance treatment adherence and connect patients with care providers
  • Create partnerships with local organizations to deliver culturally sensitive health activities.

For national health authorities:

  • Align national communicable disease prevention/management frameworks with WHO targets
  • Allocate resources to strengthen health systems, particularly in rural and underserved areas
  • Promote and implement UHC to ensure equitable access to diagnostics, treatment, and care
  • Invest in research and development for effective diagnostics, treatments, and vaccines
  • Engage with global organizations around funding, technical support, knowledge exchange

For regional health authorities:

  • Scale up regional initiatives, ensuring alignment with global communicable disease strategies
  • Strengthen regional laboratory networks/diagnostic capacities for prompt disease identification
  • Build capacity among regional health organizations to manage endemic and emerging diseases
  • Integrate environmental health strategies into regional disease prevention policies/programs
  • Develop emergency response plans to address potential communicable disease outbreaks
